TERMS AND LIMITATIONS OF THE PLANS
- This is a dental discount plan and is NOT dental insurance. It cannot be combined with any other dental insurance.
- The plan is only good for Balanced Dental services. Therefore, If you are referred to a specialist, they will NOT offer this discount.
- Treatment for dental injuries covered by workers’ compensation, disability insurance lawsuits, or outside medical care are not covered under this plan.
- This plan is NON-Transferable: Family members cannot be substituted in for another family member.
- Family members must reside at the same household address.
- This plan is NON-Refundable: No refund given if patient chooses not to use their dental plan.
- Rates are subject to change annually.
- Payments for services are due at the time of service.
- If you choose to extend your payment for treatment by paying through CareCredit®, the discount is reduced to 10% due to merchant fees.
- This cannot be combined with any other offers.
- Dental services only, products are not included.
- Discount plan not applicable to orthodontic treatment.
